42. A Computer-based Documentation and 
       Data  Management System of Clinically Relevant 
      Parameters During Repeated anti-VEGF  or 
      Steriod Injections

Purpose: To  present a comprehensive electronic medical record for documenting key features  in patients receiving intravitreal injections.
Methods: The electronic Bonn record  form is a customized data entry template with discrete data elements, graphic  presentations, space for narrative flexible text fields summarizing all  relevant parameters such as: visual acuity, optical coherence tomography (OCT),  fluorescein angiography, biomicroscopic findings and treatment data of  sequential visits. One column is designed for each separate visit.
Results: The template is based on a  Microsoft Access database with customized data input frontends, allowing  electronic storage for further statistical and graphical analysis.  The  program is also linked to the Heidelberg EyeExplorer permitting review of all  digital images of the Spectralis HRA+OCT directly onto the same screen. The  template operates in 11 languages, establishing a unique international platform  allowing for the comparison of results from different continents.
Conclusion: The structured  ‘electronic Bonn record form’, highlights the essential key values of the  patient visit from baseline through current examination and makes it available  on computer screen; thus decreasing the time and workload, while increasing the  efficiency of the time spent in patient evaluations. The novel ‘electronic Bonn record form’ will  guide the physician though the algorithm of decisions for or against further  injections.
