8. NSAID for Retinal and Anterior Segment Diseases

Purpose: To review current NSAID  options and practice in ocular disease and retinal edema.
Methods: Literature review and basic  science presentation.
Results: A brief history and  overview of NSAIDs in ophthalmology, comparing molecular structure to proposed  mechanisms of pharmacologic action is given. Methods of comparing potency are  discussed. A critical review of Phase III trial conclusions is presented.  Theories of efficacy and clinical utility for the leading agents are presented.  Emphasis is placed upon therapeutic utility for Ketorolac, Nepafenac and  Bromfenac. Conflicting Claims for these agents are evaluated. Discussion of the  systemic use of NSAIDS in eye disease and surgery is also presented.
Conclusions: Options are expanding  for the use of topical NSAIDs in anterior segment and retinal disease. This  presentation will hopefully aid the clinician in selecting the most appropriate  NSAID for his particular clinical indication.
